What kind of care is offered?

Far more than many realize. Care for early chronic conditions, home health, physical therapy and more serious care. Coordinating care plans, support for family caregivers, medications, equipment, even travel to appointments. And when needed, hospice and counseling, even long after you might expect.

Who pays for all this care?

Usually Medicare or private insurance. However, we provide all care regardless of age or ability to pay. In 2019, the Chapters Health Foundation provided $2.469 million in charity care to patients in need.

Where do we provide care?

Wherever it’s needed. Often at home, care facilities or hospitals, as well as social settings like kids’ camps.

What makes your care so special?

Too often, no one expert has the total picture of a patient’s care or possible needs. We coordinate it all, providing the best care, at the right time and place.

How long do you provide care?

Sometimes, several years. An early diagnosis or chronic condition can touch many stages, even repeatedly. Regardless, we’re there as long as we’re needed.

I am a physician. How do I determine hospice eligibility for my patient(s)?

Ultimately, hospice eligibility is centered on a physician’s clinical judgment. If a physician believes the patient will likely live six months or less if the disease follows its natural course, then the patient may be considered hospice eligible. Eligibility is based on the severity of the patient’s primary diagnosis (the diagnosis contributing most to terminality), as well as secondary/comorbid conditions and nutritional/functional status. In some cases, the primary diagnosis alone is sufficiently advanced to make a patient hospice eligible. For more information, see Hospice Eligibility Guidelines.
